Great place with full care for loved ones struggling with Dementia and Alzheimer's.
I would have my loved one living here in a heartbeat. The dementia and memory-centred care is so beautiful, the loving staff, and just in general, how this place is run, is only for the benefit of the residents who live there. In the last eight months of my mother’s life, she found relationships and felt loved in this place. The staff communicates with family members well; if I could give more than five stars, I would.
One Kenton Place is one of the best Communities for those individuals with dementia. The team is very knowledgeable and supportive.
Nine months ago we were searching for a residence for my 96-year-old father who was struggling at home alone with dementia. As a family, we decided that despite daily PSW support it was no longer safe for him to continue living on his own. I consider myself blessed that we contacted One Kenton for a tour and a meeting with their administrative staff. We were assured that his physical and mental well-being was their first priority and I can confirm that our experience has been nothing short of excellent and has enhanced and likely extended my father’s life despite living with this wretched disease. Their daily health and safety protocols kept residents safe throughout the pandemic and I believe have kept him more ambulatory and healthy than he would have been living alone at home. Daily activities, exercise and nutritious meals tailored to his needs have kept him engaged and supported in a safe environment. It is an extremely clean, bright and positive space for seniors. It is also a supportive environment for families trying to understand the challenges of the disease and support their aging family members. The staff is empathetic, cheerful, and have an abundance of patience and understanding for seniors coping with complicated health issues. I would highly recommend this facility for family members who want their loved one in a safe and supportive residence with staff that are committed to helping their residents live their best lives.
Located in a residential neighborhood across the street from a park, 147 Elder St. provides a home like environment exclusively for dementia care. In our community, dementia is the rule not the exception. It’s just life. Working with elders experiencing dementia for over 20 years, we know that there is virtually no situation in which their condition can be called stable, simple or predictable. Our skilled nurses can distinguish between emerging medical problems and evolving behavioral changes ensuring all our elders get the care they need. Every member of our community is well-known, honored, respected and loved.
At One Kenton Place we offer innovative, individualized care for those affected by Alzheimer’s disease and/or dementia by identifying a person’s social, emotional, physical, intellectual and spiritual needs. Our compassionate team of specialists and nurses, are committed to celebrating the spirit and uniqueness of each of our residents in an environment anchored by dignity, respect and the ability to thrive. Our focus is on maintaining one’s independence by building upon current cognitive and physical capabilities, while working to strengthen and develop additional skills. We empower our residents to reach their potential through encouraged independence. We ‘see’ the person and recognize that who they are is distinct from the symptoms of their disease.
With over 20 years of experience in providing dementia care, Sagecare stands unique as one of the few purely dementia care retirement homes in the GTA. Everything from our physical layout and lighting to our holistic approach to care and therapeutic activities is tailored to those living with dementia.
Our residents spend their days on our main floor enjoying therapeutic recreation activities, exercise programs, music therapy, art therapy, and a wide range of other programming tailored to their abilities. Our experienced staff partner with family members to provide truly person-centered care and programming.
What sets our community apart is our commitment to a person-centered model of care, which truly prioritizes the individual needs and preferences of each resident. Families appreciate that we offer specialized therapeutic programming designed to enhance both physical and emotional well-being, ensuring that each resident can engage in activities that resonate with them.
Our brand new building is not just aesthetically pleasing; it's equipped with state-of-the-art amenities that promote comfort, safety, and social interaction. From inviting common areas to private living spaces, every detail is designed to foster a sense of community and comfort.
Exceptional care is at the heart of what we do.
Prior to move in Sagecare works extensively with families to better understand the future resident. Families help to establish day-to-day routines, preferred foods, activities, and music, and known triggers. This information is used to structure the resident's daily routine and activities.
Upon admission Sagecare provides overnight a one to one personal support worker overnight for the first two nights to better understand the resident's sleep patterns and ensure they're safety.
We understand that visits can be difficult for families during the transition period. Our team facilities visits with the new resident to help both families and residents feel more at ease with the move.
As residents adjust to life in our retirement community, we offer a comprehensive range of support and resources to ensure a smooth transition. Our dedicated team includes a Director of Care, Assistant Director of Care, a doctor, nurse practitioner, nurses, and PSWs who work collaboratively to provide personalized attention and address any health-related needs. In addition to our exceptional medical team, our recreation and occupational therapy teams play a vital role in helping residents acclimate. They design engaging activities and programs that encourage socialization and connection, fostering a sense of belonging right from the start. Together, these teams create a holistic support system for our new residents.
Because dementia is progressive and unpredictable, our care plans are constantly evolving, adapting daily routines, therapies, and approaches to meet each resident where they are. Our low staff-to-resident ratio allows us to respond quickly and personally, while maintaining consistency and familiarity. Families are closely involved throughout, so care remains aligned with the resident’s history, preferences, and evolving needs.
In our community, we prioritize the evolving needs of our residents, ensuring they receive the appropriate level of care as their requirements change. If a resident’s needs increase, our dedicated team works closely with the individual and their family to assess the situation and develop a tailored care plan.
One of the standout features of our community is that residents can remain in their current suites even as their needs change or progress. We offer options for suites specifically designed for those requiring higher care levels, making it easy for residents to transition within our community if they choose. We welcome outside support services if needed.
Sagecare is designed exclusively for individuals experiencing cognitive decline, where specialized care, structured routines, and therapeutic programming are part of daily life. We’re best suited for families seeking a highly focused, relationship-driven approach to memory care, rather than a general retirement setting. Someone who needs light assistance or who is fully independent will be better suited to one of the numerous excellent retirement homes in our contact network.

Sagecare recognizes that dementia has tremendous effects on the family of the resident and they require support as well. Sagecare has an onsite social worker who works with families, holds monthly support groups, and provides one-to-one therapy and counselling to our families.
Our core team of nurses, personal support workers, dietary staff, and housekeepers have been with Sagecare for over 10 years. It's this experienced team of staff that helps take the care element away from families and give them the opportunity to be a loved one.
We also have a robust family council who helps our new families with integrating into our community, and supports them through the transition.
Many newcomers initially expect a typical retirement home environment, but they quickly learn that our community is more like a close-knit family. From the moment they arrive, it's evident that we foster a welcoming atmosphere where everyone—residents, staff, and family members—plays an integral role in creating a vibrant community. We encourage families to participate in our special events and daily programs, allowing them to engage and build relationships not only with their loved ones but also with other residents and staff. Our calendar is filled with engaging activities, social events, and opportunities for collaboration that go beyond traditional expectations.
We’ll continue to build on over 20 years of specialized dementia care, refining our person-centred approach as best practices evolve. That includes expanding tailored environments, like our newer memory care spaces for different stages of dementia, and continuing to invest in staff training and therapeutic programming. Our focus will remain the same: creating a home where residents are understood, families feel supported, and care adapts thoughtfully to each stage of the journey.
As we look to the future, we envision our community continually evolving to enhance the quality of life for our residents through innovation and cutting-edge technology. Our mission to provide compassionate retirement care without compromise guides every decision we make, and we are dedicated to integrating new treatments and advancements that support personalized care. Our focus on always changing and getting better means we will continually assess and adapt our programs and services based on family feedback and emerging best practices in senior/dementia care. This commitment not only aligns with our vision of enhancing residents’ lives but also creates a dynamic environment where everyone feels valued and empowered.
